feat(fuzz): group saved crash reports

Saved fuzz reports with the same stack have no shared triage identity. This adds private grouping by the top five native or Python frames and separates timeout and out-of-memory reports, as requested in #1019.

Run python tools/fuzz/fuzz.py --mode triage report.log --output triage.json to group saved reports. The command keeps frame details in the output file and prints group counts. Structural reduction and the MSan/TSan lanes remain requirements for completing #1019.
