Bump Cargo.toml, Cargo.lock, and herdr-plugin.toml together so the displayed plugin version matches the crate. Cut the Unreleased changelog into 1.16.0 (pinned preview, changed_file_view, skill --cwd fix) and point the pinned- preview docs-consistency check at that section. Also land the front-door README refresh and the pinned-preview screenshot so the release notes and the storefront match.
