Add outreach-sequencer skill

Summary

Adds the outreach-sequencer runx skill for Frantic #74.

The skill reads a bounded outreach sequence definition plus durable engagement projection, decides whether the next touch is eligible, appends an ungated data-store decision event, and emits a handoff-only runx.outreach.next_touch.v1 packet for a separate governed send-as run. It never sends outreach directly or mints dispatch authority.
