fix: resolve failing backend tests and dynamic import issues

• Fix index.test.ts: update countAllEvents assertion to match actual 3-arg signature
• Fix integration.test.ts: replace dynamic imports in claim handler with static imports to avoid module reset issues after vi.resetModules()
• Fix integration.test.ts: insert mock stream into fresh app's DB for rate limit test
• All 554 tests pass, TypeScript build succeeds
