Implement Stacker News GraphQL social adapter

Summary

• replaces the social-stackernews placeholder with real Stacker News GraphQL posting
• uses the live upsertLink mutation for link posts and upsertDiscussion for text posts
• supports territory selection, custom cookie key, and API URL override for tests/self-hosting
• maps PayIn/item responses into sh1pt PostResult values and surfaces GraphQL errors
