Selected #14852 because it was the only unassigned medium_effort curation item with no open PR and no non-agent assignee (others already have PRs or are decision threads).

Re-fetched PMID:29726930 as full text (cache was abstract-only). Added: PRIMARY_RESULT quote for absent thymus/no functional T cells, an allogeneic thymic transplantation treatment (CELL_THERAPY, NCIT:C15289 since NCIT has no thymus-specific term; no efficacy claim as the main text gives no outcome), and updated the notes. The phenotype stays at HP:0002721 because the report gives no numeric T-cell counts (they are in the supplement, unavailable).

Validation: validate, validate-terms, snippets 46/46, entity refs, history all pass.

Review: VETD (TBX2), full text of PMID:29726930

Verdict: changes requested. The new evidence is accurate. Both new quotes appear word for word in the full-text cache, and the reworded immune description (Individual 2 has low T-cell numbers, Individual 3 has low naive T cells) matches the paper's results paragraph. Two things need fixing before merge. The new thymic transplant treatment is not connected to the pathograph. It is also tagged with a different modality from the same treatment in the two other entries that carry it.

  • Schema validation would pass (the author reports validate and validate-terms pass)
  • All PMIDs verified as real (PMID:29726930, re-fetched as full text; the cache diff only changes frontmatter and adds body text)
  • Evidence snippets are exact quotes (both found in references_cache/PMID_29726930.md, line 88)
  • Ontology terms correct and specific (NCIT:C15289 is what sibling entries use for this procedure)
  • evidence_source classifications correct (HUMAN_CLINICAL + PRIMARY_RESULT, correct for the proband description)
  • No pathophysiology bundling

Reference titles: 2 new evidence items, both carry reference_title, and both match the cache frontmatter. The paper is the defining report for this disease.
Scope: 3 files, all in scope. No sign of a bad conflict resolution.

🟡 IMPORTANT

1. The transplant treatment is not connected to the pathograph. Allogeneic thymic transplantation (around line 793) has no target_mechanisms and no target_phenotypes, so it renders as a node with no edges. The node it acts on already exists: Impaired Thymus Development at line 249. The two other entries with this treatment both make the link. T-cell_Immunodeficiency_Congenital_Alopecia_and_Nail_Dystrophy uses target_mechanisms: [Athymia] plus HP targets, and 22q11.2_Deletion_Syndrome uses HP targets. Add:

  target_mechanisms:
  - target: Impaired Thymus Development
  target_phenotypes:
  - preferred_term: Aplasia of the thymus
    term:
      id: HP:0005359
      label: Aplasia of the thymus

The link only says what the treatment is aimed at, so it needs no efficacy claim. The "no outcome reported" caveat can stay in description.

2. therapeutic_modality: CELL_THERAPY is inconsistent with the KB. CLAUDE.md's backfill table maps NCIT:C15289 (Organ Transplantation) to SURGERY. Both sibling entries tag this same procedure SURGERY. If you think thymus tissue really is closer to cell therapy, argue it in the PR. Otherwise use SURGERY, so that a query for thymus transplant by modality returns all three entries.

🔵 SUGGESTION

What I checked and found fine

  • Keeping the immunodeficiency phenotype at the broad term HP:0002721 is still justified. The main text gives no T-cell counts, and the description now says so accurately.
  • The top-level notes rewrite drops the old open question about the UDN web-page claim, because that claim is now cited to a publication.
  • No research artifacts are touched, so the deep-research completeness cross-check does not apply to this narrow edit.

Both IMPORTANT items are small edits. Push them together with the history links in one commit.

Requesting changes. Both new snippets are exact quotes from the PMID:29726930 full-text cache, and the reference titles match the cache. Two IMPORTANT fixes are needed. (1) The new 'Allogeneic thymic transplantation' treatment has no target_mechanisms or target_phenotypes, so it is not connected to the pathograph. Link it to 'Impaired Thymus Development' and HP:0005359, as the 22q11.2 and FOXN1 nude/SCID entries do. (2) therapeutic_modality CELL_THERAPY disagrees with both sibling entries and with the CLAUDE.md table, which map NCIT:C15289 to SURGERY. Suggestions: add NCT00579709, which the paper names (cache line 128), under clinical_trials; fill history links with #14852 and #15101. The full review is in the PR comment.
