Timing Ph2: measure what's missing #218

Phase timing from #308 gives per-iteration planning_ms, execution_ms, task_compute_ms, and tool_ms, but execution_ms deliberately folds worker waves, persistence drain, and result consolidation into one number (orchestrator.rs:3672). Still unmeasured: MCP init, create_worker, wave-barrier idle, session-history handling, and synthesis.

This is now the gating ticket for the two surviving performance questions. #224 needs per-wave completion timing and #225 needs init-phase timing. The 2026-07-17 vetting pass closed the other Epic 4 items on measurement, so what remains of the performance epic hangs on this instrumentation.

    Still needed after #308 (checked 2026-07-17): execution_ms folds waves, drain, and consolidation into one number by design, and nothing measures MCP init, create_worker, wave-barrier idle, or synthesis. This ticket is now also the gate for settling the #224 and #225 magnitude questions, which survived vetting today on exactly that missing data. Description updated.
