    Automated triage could not produce a review

    The pulse attempted to post an automated triage review on this issue 1 time(s) but every attempt was suppressed by the safety filter. The content-hash cache has now been written to stop the lock/unlock churn, which means this issue will no longer appear in the automated triage queue until its body or comments change.

    What went wrong

    • Reason: Worker produced output but it did not contain a ## Review header (safety filter suppressed the post).
    • Last attempt output size: 1240 chars
    • Failure tag: no-review-header

    What a maintainer should do

    Throughput recovery evidence request

    PR #29235 merged the conservative Pulse ceremony default: unset or invalid DISPATCH_MAX_PARALLEL now resolves to min(6, effective_slots) while explicit overrides and serial throttling remain intact.

    This dashboard last refreshed at 2026-08-02T03:07:14Z and still reports 0 active workers, a 2-worker maximum, 25 worktrees, healthy memory, and moderate load. After auto-update deploys a runtime containing merge d83983983697, please post one compact evidence snapshot with:

    • deployed SHA and whether DISPATCH_MAX_PARALLEL is explicitly set;
    • pulse-current-state-helper.sh --window 15m diagnosis;
    • the configuration/provider constraint producing max_workers=2;
    • one post-update Pulse cycle's available slots, attempted candidates, spawned workers, terminal launch failures, and host load.

    Do not raise the worker maximum solely for this request; the goal is to distinguish intentional capacity from a stale or hidden cap before changing it.
