Clean up the v2 issue activity markers so app-backed activity does not get a colored avatar ring, while user-backed activity keeps a lighter ring. Also makes escalated activity a darker warning line icon and keeps completed Seer activity neutral. Co-Authored-By: Codex GPT-5 <noreply@openai.com>
