feat: add JSON and YAML output formats for all commands - #65

- Add global --output flag supporting table (default), json, and yaml formats
- Create internal/formatter package for output formatting
- Update all commands (version, health, whoami, list-topics, usage) with output
- Add YAML tags to response structs
- All formats work with --debug and --disable-auth modes
