fix(submodules): remove orphan submodule references

Description

Fixes inconsistent submodule references in the monorepo.

Some orphan submodules (.claude, bms-monorepo, and evo-ai-docs) were still registered in the Git index but missing from .gitmodules, causing commands like:

git submodule
git submodule foreach
git submodule update --init --recursive

to fail with errors related to missing submodule mappings.

This PR removes the orphan submodule references and cleans the repository state so submodule operations work correctly again.
