Add one refresh action for the chat menu and Providers tab, and keep stale chat selections safe until the server resolves them.
Ignore superseded provider list replies, clear stale variants when a model falls back, and keep multiple refresh warnings visible.

Context

Emacs users need a way to request the server's current model catalog without restarting ECA. A refreshed catalog also needs to leave the Providers tab and active chats in a consistent state when models disappear.
Changes
eca-chat-refresh-modelsto the chat menu and Providers tab. Refresh the tab's provider counts after the server responds.providers/listresponses so an older reply cannot replace a newer tab view.*Messages*and show a final warning count when more than one provider reports a problem.
