Resized form inputs

Resized the form inputs on the apply and teach page to look better.

Programming test now exist on the apply page.

Previously, the programming test used to be linked to, wasn't opening in a new tab. Now the significant text of the programming test has been added directly on the apply page above the <textarea> where the code should be pasted.
