Update the README to reflect the current plugin support and distribution model.

Added GitHub Copilot and Codex CLI to the supported plugin marketplaces.
Updated the "Commands and hooks" section to describe provider-specific behavior.
Clarified how plugin marketplaces distribute provider bundles.
Improved the documentation to match the current implementation.
