feat: scope the links widget to the narrowed region (#68) - #71

With the buffer narrowed, the links widget counts only links positioned inside the restriction, with the same narrowed marker on the header count as backlinks. Positions come from the database, so unsaved edits can shift them until the next sync. Also documents a recipe for refreshing the sidebar instantly on narrow/widen by wrapping org-toggle-narrow-to-subtree.

The links widget now consumes the narrowing scope context: with the buffer narrowed, only links positioned inside the restriction are counted, and the header count gets the same narrowed marker as backlinks. Filtering is by link position rather than by owning note, since the file-level note's link list spans the whole file. One caveat, stated in the docs: positions come from the database, so unsaved edits can shift them until the next sync.
Also adds a README tip for refreshing the sidebar instantly on narrow/widen by wrapping org-toggle-narrow-to-subtree (thanks @alexjp).
