Adds the Day-3 session record for the daily-use evidence window opened on 2026-08-01. Documentation only: one new file, docs/operations/daily-use-evidence-window-2026-08-06.md, 350 insertions. No code, test, or configuration change.

What it records

One eligible real tc run execution — Trial 011 — against merged main @ 3f1cce0, no configuration modified at any point. Completed on local_fast with qwen2.5-coder:7b-triagecore at fallback_depth 0, full token/elapsed telemetry (937 tokens, 27.24s).

The task reviewed the Next Gate correction (1546776, then unmerged) for temporal accuracy and claim boundaries. Operator disposition: rejected — the window's first. The output was structurally compliant (findings-only, as requested) but substantively wrong: it called a true historical statement false, inverted an explicit authority disclaimer into an authority implication, read a baseline requirement as a readiness claim, and its one correct observation concerned text the diff had already deleted. No true finding affecting the merge decision was produced; the correction was reviewed and merged on the operator's own independent verification instead.

Two governed previews preceded the attempt and are recorded as session context/friction, not eligible attempts. The first blocked fail-closed (privacy_preflight_failed) because the input was built with plain git show, which carries the commit's Author:/Co-Authored-By: metadata — both matched the scanner's email grammar. That's a correct control response to an over-broad input, not a scanner defect; fixed by rebuilding with --format= --no-color --no-ext-diff.

Cumulative window state (3 distinct days: 2026-08-01, 2026-08-02, 2026-08-06)

9 eligible executions, 4 completions, 0/9 validations run, and — across the four completions — all three possible operator dispositions: 1 accepted, 2 revised, 1 rejected. That distribution is the headline finding: route success, generation success, validation, and operator acceptance are empirically distinct outcomes, demonstrated on real runs rather than assumed.

Sequencing and temporal consistency

Third of three related documentation PRs, merged last by design: PR #147 (Day-2 record) and PR #148 (Next Gate correction) merged ahead of this one, in that order, so this record's citations resolve on main. The record explicitly distinguishes session-time state (both were unmerged when Trial 011 ran) from current repository state (both now merged), without altering Trial 011's baseline — it ran against main @ 3f1cce0 and reviewed the correction as an unmerged candidate at that time; the rejected disposition rests on the review's content, not the correction's later merge status.

Claim limits

Makes no reliability, validation, correctness, or readiness claim, recommends no implementation change, and grants no authority.

Verified before opening: rebased and squashed onto current main (4f3eca1, after PR #147 and #148 merged); single commit, one new file; repository document-reading guards (test_governed_decision_integration_absence.py, test_propose_cli.py) pass, 12 tests; staged whitespace check clean.

Generated with Claude Code

Records one eligible real tc run execution, Trial 011, against merged main
at 3f1cce0 with no temporary configuration and no configuration change.
Trial 011 completed on local_fast through the committed CR-DD-014 binding
with full token and elapsed telemetry, and the operator disposition is
rejected: the output was structurally compliant but produced no actionable
true defect and several consequential false positives. It is the window's
first rejected disposition.

Two governed previews are recorded as session context and friction rather
than attempts. The first blocked fail-closed because the input artifact was
built with git show without --format= and carried commit metadata matching
the scanner's email grammar; no model call, no ledger write, and no privacy
control was weakened or bypassed.

Window totals depend on the Day-2 record at f891fcc, which merged into main
via PR #147, and cite the Next Gate correction at 1546776, which merged via
PR #148 -- both merged before this record was opened as a pull request, in
that order. The record distinguishes session-time state (both branches
unmerged when Trial 011 ran) from current repository state (both merged),
without altering Trial 011's baseline: it ran against main @ 3f1cce0 and
reviewed the Next Gate correction as an unmerged candidate at that time.
Cumulative figures: 3 distinct days, 9 eligible executions, 4 completions,
0 validations, and dispositions of 1 accepted, 2 revised, 1 rejected.

Day-1 derived findings 11 and 12 are recorded as still unresolved; finding
13 is recorded as superseded by Day-2, not by this record.

Documentation only. No code, test, or configuration is changed, and the
record makes no implementation recommendation and grants no authority.
