gum table interactive mode renders blank rows (header/footer only): regression in v2.0.0

Environment: macOS, Kitty terminal (TERM=xterm-kitty, COLORTERM=truecolor), zsh

Description:

After upgrading to v2.0.0, gum table's interactive mode renders the column headers and the footer help line (1/1 · ↓↑ navigate · enter select · esc quit), but the data rows themselves are completely blank: no borders, no cell text. This happens regardless of:

• number of rows (reproduces with 1 row and with 2 rows)
• --height (default auto and explicit --height=3)
• --widths (default auto and explicit)
• selection color overrides (--selected.foreground, --selected.background)
• input source (stdin pipe vs --file)

gum table --print (non-interactive) renders the same data correctly with full borders. gum choose (interactive) also renders correctly. This isolates the regression to table's interactive Bubble Tea view specifically.

Reproduction:

printf "ID,NAME,STATUS\n1,Alpha,Active\n" | gum table -s ','

Expected: an interactive table showing the one data row, selectable.
Actual: header row and footer help text render, but no borders or row content appear between them.

Confirmed working (for comparison):

# gum choose: works, shows all options with cursor
