Pi could create a hosted pull request without one final confirmation, which is too risky for a write tool. The extension now uses Pi's native dialog, presents the payload as a readable review card, and fails closed when UI is unavailable or approval is denied. Untrusted fields are sanitized before terminal rendering. OMP and MCP already expose the operation as a write.
