feature(pool): add SEP-41 decimals()/name()/symbol() metadata for LP shares

Summary

SEP-41 requires metadata functions (decimals/name/symbol) so wallets can display the LP share token correctly. These should be set once at initialize() time per pool instance, consistent with the per-asset pool_factory model where each instance's share token should be named after its underlying asset.

Acceptance Criteria

• decimals(env: Env) -> u32, name(env: Env) -> String, symbol(env: Env) -> String added to contracts/pool/src/lib.rs
• initialize() gains name/symbol parameters (or derives them from the funding_asset address), stored in new instance DataKey entries
• Test added/updated and passing (cargo test)
