This pull request adds the instance::attribute and body::attribute columns suggested by Yaw in this thread (bind::attribute was already supported): https://groups.google.com/forum/#!topic/opendatakit-developers/hgAXcRBkmAs
Also, ${variables} are supported in the attribute columns.
