Summary

• bound automatic review and implementer repair at three unresolved failures
• require new evidence, context, or a strategy change before retrying, with a focused retry brief
• sync manifests, install docs, runtime markers, and changelog to v1.0.1
• keep the existing DP-5 debugging ledger unchanged; no new configuration or control layer

Release boundary

This PR prepares the source release only. It does not create a tag, publish npm, or create a GitHub Release. The external Codex marketplace must report v1.0.1 before the release tag is authorized.
