Medication Overview ...

Medication Treatment A set of 1..n medication treatment lines that are somehow related together (the same purpose, same entry by the a professional, or other)

Medication Treatment Line An entry representing one single medicinal product (or type of product) that is planned, or ordered, or dispensed, or reported used by the patient.

Verified medication overview Treatment lines that have been verified by HCP as one treatment plan for the patient. The HCP takes responsibility of potential interactions.

Verification of treatments A health care professional verifies, i.e takes responsibility of the full medication overview or the lines s/he is authorised to take responsibility of. Verification is done on full overview, but the information of having been verified is stored separately for each treatment line, as it is possible to have new lines that are not verified yet. The time of verification may be the time of update of the schema - the collection of verified treatment lines becomes a document with an author and time of update. In business process, medication verification consists of requesting a supply overview from the community pharmacy (all sources prescibers, pharmacy, etc.), conducting a medication admission interview by a health care professional and drawing up an up-to-date medication overview (adding new medication treatment line).

Patient statement Patient can state taking a medication that has not been prescribed, or state NOT taking a prescribed medication. Patient statements have an impact on the medication verification process, the statements should therefore be included in the verified overview even when not conceptually agreed by the HCP.

Terms we do not use

Medication Treatment Plan is a subtype of medication treatment line or order, that is in planned status or has not been executed yet. It is an ambiguous term and better avoided.
