feat!: rename ai-readable-content factor to ai-access-files#49

Rename the factor scoring root-level AI access files (llms.txt, llms-full.txt, robots.txt, sitemap.xml, per-page Markdown endpoints) from ai-readable-content to ai-access-files, display name "AI Access Files (llms.txt, sitemap)". BREAKING CHANGE: factor id, all 20 finding codes (ai-readable-content.* -> ai-access-files.*), the --factors ai-readable-content value, and the analyzeAiReadableContent export are renamed. schemaVersion bumped 2.1 -> 3.0 to flag the break (report shape unchanged); package version -> 4.0.0.
