provide form field in OOD apps to load modules

We've discussed this before and this is possible in the Code Server (slurm-based) app, but I continue to think this would be useful for other apps.

As an example, in INC1744399, a user needs to load hdf5 in order to use the R package hdf5r.

@wfeinstein could you let me know your thinking on this topic now that some time has passed since we set up the functionality for VS Code?

Another example of where this would be useful is for loading the gsl module for use with a package that the user in INC1755935 needs.
