Infrequently, at random:
"Oops, something has gone wrong (Err9823). Please report this error to your system administrator and try again"
Then delete the current password, reset, and start all over again
Hope they know who their system administrator is

Forcing the user to retype the password on each try should be mandatory even without that inconvenient mishap … ;-)

Possibly. I was going to suggest that, but thought that it might make people bail out too soon. I'll add it as a separate issue for discussion anyway
