Add mgw:assign command — claim issues and update board assignment

Description

New MGW command: mgw:assign [username]. Claims an issue for the specified user (defaults to @me): assigns via gh issue edit --add-assignee, updates the board item Assignee field, and writes the assignee to the .mgw/active/ state file. If the issue is not yet in .mgw/active/, creates a minimal state entry. Emits a confirmation with the board item URL. Follows delegation boundary — only state and GitHub operations, no code reads.
