Sync linked PRs and branches onto board items via cross-refs

Description

When mgw:pr creates a PR or mgw:link records a branch cross-ref, update the board item with the PR URL in a dedicated PR Link field. Implement in pr.md and link.md post-creation hooks. If the board field does not exist, create it. Allows teams to navigate from board to PR without visiting GitHub issues separately.
