Surface AI Agent State field on board items during GSD execution

Description

When mgw:run or mgw:milestone spawns a GSD agent, write the active agent type and GSD route to the board item AI Agent field (e.g., 'plan-phase: gsd-planner', 'execute-phase: gsd-executor'). Clear the field when the agent completes. Store the board item ID in .mgw/active/.json as board_item_id for fast field updates without re-querying. This gives team members real-time visibility into what the AI is doing.
