Document /mgw:ask and /mgw:review in README

Summary

The README commands table and the Verify block are missing two deployed commands: /mgw:ask and /mgw:review. Add them to maintain documentation accuracy.

Acceptance Criteria

• Add /mgw:ask to the commands table with description and usage example
• Add /mgw:review to the commands table with description and usage example
• Update the Verify block (line ~251) to list the correct file count (14 files, not 12)
• Verify the ls output matches actual deployed files in ~/.claude/commands/mgw/

Context

• /mgw:ask — Contextual question routing during milestone execution
• /mgw:review — Review and classify new comments on a GitHub issue since last triage
• Both commands are fully implemented and deployed but were added after the README was last updated
