Add Who This Is For and troubleshooting sections to README

Description

Add a Who This Is For section after the architecture overview: solo developers and small teams using Claude Code who want GitHub to reflect their actual work without manual project management. Add a Troubleshooting section at the bottom: common issues (gh auth status, GSD not installed, .mgw/ state drift, rate limiting), and uninstall instructions (npm unlink, remove ~/.claude/commands/mgw/, remove .mgw/). Keep both sections concise.
