Write USER-GUIDE.md covering: .mgw/ directory structure and what each file does, config.json options, how to customize issue/PR templates, how to use --dry-run and --interactive flags on milestone, how dependency ordering works (Kahn's algorithm, blocked-by labels), how to recover from pipeline failures (re-run, sync, manual state edits), how to use MGW with existing GSD projects, and how cross-references work. Target audience: someone who has installed MGW and run /mgw:help but wants deeper understanding.
