Need different icons for 'Log Out' and 'Log Out...' #189

Presently we have system-log-out appear suitable for 'Log Out...' at 16 and 24 pixels and then 'Log Out' at larger sizes

and this causes problems in the whisker menu when it shows icons of 32 pixels or larger, as it shows the incorrect icon for 'Log Out...'

no difference between the 'Log Out' and 'Log Out...' icons in the whisker menu commands area.

Update session icons (logout, shutdown, etc)
70f6d45 
Made logout icon same look at all sizes (Fixes shimmerproject#189)
Changed shutdown icon colors to match newer flatter elementary style
New suspend and hibernate icons (w/assets from xfce4-session icons)
New user switch icon
Delete duplicate icons from `apps` folder
Delete non-matching icons at higher sizes
(I don't see a place where they are used,
xfce-session uses icons only up to 48px)
