How Do I Fix When Full Name Fills First Name Field?

In many online forms, my full name fills the first name and last name fields. How do I fix this?

Hi,

Please see attachment to view the error. As you can see, both my first and last name fills both First name and Last name fields. Let me know what you suggest. Thank you.
