can you check logs while vagrant VM is installing? Maybe you have:
Warning: /Stage[main]/Mysql/Service[mysql]: Skipping because of failed dependencies

This is problem with puppet, you can use puppet module ( [https://github.com/puppetlabs/puppetlabs-mysql] ) or you can manual install mysql server in these steps:

  1. open ssh: vagrant ssh
  2. install run: sudo apt-get install mysql-server
    enjoy!
