Summary

Moldervine Reclamation did not trigger after the controller sacrificed a creature token using Phyrexian Altar.

Expected behavior

Whenever a creature controlled by Moldervine Reclamation’s controller dies, including a token that is sacrificed, its life-gain and card-draw trigger should be created.
