Update the couchdb image version defined in docker compose

The image klaemo/couchdb:1.6.1 referenced in the docker-compose.yml is deprecated and one gets prompted to use the official CouchDB image https://hub.docker.com/_/couchdb/

• Reference an image that is a later 2.x Apache CouchDB version

• Update the corresponding documentation to remove steps on setting up couchdb-lucene as this is no longer required with Postgres being used as the main database when setting up OpenSRP using Docker
