Add URLs of ValueSets of DICOM CIDs to index page

Add the direct URL to the FHIR ValueSet from the official FTP of dicom CID tables specified in CP-1570 to the index.html page, so they can easily be accessed/found.

These could allow developers to build the DICOM also from drop downs, by querying this resource, or importing it into their software.

No, this is already present in DICOM: all CID tables are in FHIR already on an ftp server. dicom4ortho should have an issue to pul them from there. We could link them to the index.html page... I'm changing the title to reflect this pivot.
