cleanup: remove redundant MERMAID_STANDARD.md file

- Delete docs/MERMAID_STANDARD.md as Mermaid standards are already documented in .agent-os/standards/code-style.md
- Remove MERMAID_STANDARD.md references from workflow paths-ignore filters
- Update .cursorrules to reference Agent OS location for Mermaid standards
- Update CI/CD documentation to reflect simplified path filters
- Consolidate all standards documentation in Agent OS structure for single source of truth
