otel moved to using rack events
https://github.com/open-telemetry/opentelemetry-ruby-contrib/tree/main/instrumentation/rack#rack-middleware-vs-rack-events

and it seems we have orphan spans now, so test and see what to do.

tested latest rack instrumentations and output seems fine
