Mouthwash survey - update functionality to keep survey open based on response #1198

We would like for the mouthwash survey to function similarly to the way the menstrual cycle survey currently functions.

Please see GitHub Issue in Questionnaire repo: episphere/questionnaire#439

[667908442] "Please answer this survey only after you've collected your home mouthwash sample. If you haven't collected it yet, close the survey and return it after you've collected your sample."

If participant responds [668770150] "Yes, I already completed my home mouthwash survey" - they will move through the survey as normal

If participant responds [141992510] "No, I will return to this survey after I have completed my home mouthwash sample donation" - they should be brought to the End Message, "Thank you. When you have completed your home mouthwash sample donation, please return to complete this survey."

After the end message, the participant will see a Box that says "OK" and the survey will exit and clear their cache so the survey remains on their MyConnect Dashboard unanswered.
