Add JIRA env to terraform

What

Add the following environment variables to the staging and production configurations:

JIRA_URL=your_jira_url
JIRA_BASIC_AUTH=your_jira_basic_auth
JIRA_SERVICE_DESK_ID=your_jira_service_desk_id

Why

We need these environment variables for the integration of the JIRA endpoint form submission.

Note: API keys will be generated and shared securely.

@gmatchett - the Endpoint Submission form is something we have been trying to get over the line for sometime so would appreciate if this can be prioritised for completion in a few days if possible.
