v1.6.0 released — Foundry funnel + loop-gate

Three bullets:

1. loop-init --with-foundry — design the loop and scaffold a versioned harness in one shot (loop-audit 1.7 scores Harness Runtime).
2. loop-gate 1.0 — mechanical denylist + auto-merge allowlist from gate.yaml.
3. Safer long runs — loop-context stagnation detection + loop-worktree --wait.
