Workzone LEP Configured app Giving an error at the Launchpad page

Hi Team,

I am the steps as mentioned here to deploy an LEP - https://github.com/SAP-samples/btp-developer-guide-cap/blob/main/documentation/saas/lep/1-prerequisites.md

We tried to build and deploy the incident management application using - https://github.com/cap-js/incidents-app/tree/saas-lep branch.
As mentioned we added the tenant specific route and Added the user to the Role_collection.

Build and deploy was successful
subscription was successful, The tenant specific route was also configured by me. I am able to launch the LEP URL from the subscriber account, But I get an error message as shown in the screenshot

Please help with this issue and let me know how can i resolve this issue. thanks in advance.
