Plannotator VS Code extension does not intercept plan reviews initiated by the Claude Code VS Code extension

Summary

The Plannotator VS Code extension successfully intercepts and opens plan review requests within a VS Code tab when Claude Code is run from the integrated terminal. However, this interception does not work when using the official Claude Code VS Code extension; those requests continue to open in the external system browser.

I'm glad to hear it's working at all. Previously, there were issues with how the extension did hooks in VS Code.

Now that it's at least working, I'll give it a look and see what's going on.
