How to Create a Focus Item

I need to create a flow for the user to create a "Focus Item". 

Scenario:

> I've got an hour before my next meeting, and I need to get 1 of many things done. How do I pick??? Where do I store a representation of that thing and how will I be able to remember that I even did it?
