Add copy to indicate that creating an environment cannot be undone or edited later

First pass could be a simple text update on the form label:
PHI Ready
PHI Ready environments cannot be created from existing environments. Specifying PHI Ready is not reversible.

Another option?
No checkboxes, two separate forms to really solidify these are two different environments, each with a description prior to selecting which "new" you're creating.
