[hard] Backfill verification_requests with missing contract metadata

Summary

A subset of verification_requests rows are missing contract metadata that was not captured before a recent schema addition.

Requirements

• Write a backfill script that fetches and populates missing metadata
• Add a dry-run mode reporting affected row count
• Add a test verifying backfilled values against known fixtures
• Document the backfill run and results in docs/decisions.md
