chair name and email sporadically clearing after submission creation

Vireo site Admins are finding occasional missing chair name and email values. The fields are required for submission creation and a [fix] is required when not present, but after creation the values are not always present in the UI nor the database.

I have not yet been able to replicate on demand in testing.

this issue can be replicated when an INPUT_CONTACT field is used (such as for Committee Chair) and no Controlled vocabulary is selected (even though INPUT_CONTACT does not use a CV)
