docs: clean up .codex to remove non-repository specific settings

While we need to define a repository map and some other repo-specific settings, there are some .codex settings and AGENTS.md text that are too general.

This is also a good chance to start looking at where else repo documentation can be improved/streamlined for integration with agents.
