Implement pr-template.md file

The task is to create a prtemplate.md file under .github so that contributor gets a proper pr template that consists of all pr description, issue number, changes made, checkboxes of all like codebase, no breaking like this, and at last screenshot section.
This is done in order to standardize the pr's
